This petition must be submitted to the National Board of Virtuous Ladies of God, Inc. by a group of Gems-at-Large requesting permission to establish a provisional chapter. All required signatures and attachments must be included for consideration.

We, the undersigned Gems-at-Large of Virtuous Ladies of God, Inc., hereby request recognition as a provisional chapter in our state. We affirm our commitment to the Constitution, Bylaws, and policies of the National Organization.
